The pattern was missing information regarding where to get foam pillow forms. We had ours custom cut; most local upholstery stores will provide this service. Alternately, foam from a craft store, cut to the proper shape using a serrated knife and layered to desired thickness, would also work well.

There is an error in the row count of the SLEEVE on sizes 12 (24) months only and all sizes. Note the corrections below in bold:
